Student information form
Each year we request parents complete a Student Information Form which includes the following:
- Parent/Carer details
- Emergency contact details
- Medical information
- Additional information
If there are any changes throughout the year relevant to the above information, we request you complete another form.
Long term permission note

When a student enrols at Ambarvale Public School we request that parents complete a long term permission note. This note remains in force while the student attends Ambarvale Public School and parents can request a change at any time by sending in another note. This permission note cover areas such as:
- Publishing names/photographs in schools newsletters
- Publish names/photographs on school and library web page
- Publish names/photographs in out of school publications
- Videotape and show activities at school
- Walk to and from local High School with teacher supervision
- Walk to and from and participate in sporting activities at local sporting fields, with teacher supervision (eg: Council Grassed Area, Rosemeadow Playing Fields)
- Walk around the local area/community to participate in learning activities outside the classroom, with teacher supervision
- Travel by bus to sporting venues on Gala Days in local areawith teacher supervision Attend Excursions/performances
- Access the Internet
